2001-2002 Auto Insurance Rate Guide Information Form

TDI´s auto rate guides group individual rating territories into regions. Each region consists of two to four territories. For your company, list the appropriate annual premium in effect on June 1, 2001, for PERSONAL LIABILITY INSURANCE ONLY (limits of 20/40/15) for each driver profile. If you write policies on a month-by-month or semiannual basis, multiply the monthly or semiannual premium by the appropriate number to calculate an annual (12-month) premium. Add any monthly fees you normally charge. See the Commissioner´s Bulletin No. B-0025-01 for an explanation about which companies are required to provide information for the rate guides. Company Name The company´s name. NAIC Group and Company Number The NAIC three-digit group and five-digit company numbers. Consumer Information Telephone Number The number consumers may call to get information about rates or how to get a quote. If your company has a toll-free number for consumers, please use that number. Company Contact for Additional Information The name of the person responsible for reporting the information on this form to TDI. Company Contact Telephone Number The direct phone number of the person listed as the company contact. Company Contact E-Mail Address The e-mail address of the person listed as the company contact. Driver A Annual Rate for Minimum Liability for Driver A: is a male age 25-64 or female age 21-64 with no at-fault accidents or major traffic convictions who drives to and from work. BI+PD (20/40/15), 1 year, Class 1B Driver B Annual Rate for Minimum Liability for Driver B is: a single male under age 21 with no at-fault accidents or major traffic convictions. BI+PD (20/40/15), 1 year, Class 2C-1 Driver C Annual Rate for Minimum Liability for Driver C is: a single female under age 21 with no at-fault accidents or major traffic convictions. BI+PD (20/40/15), 1 year, Class 2D Driver D Annual Rate for Minimum Liability for Driver D: is a male or female over age 64 with no at-fault accidents or major traffic convictions who drives for pleasure and errands only.
